Your First Visit

  1. If your insurance company requires a referral from your primary care physician, please be sure to obtain this prior to your appointment.

  2. Your first examination may include x-rays or other diagnostic tests. Please allow adequate time for this visit and wear loose fitting, comfortable clothing. Our office will advise you to arrive 30 minutes prior to your appointment time if x-rays are required.

  3. Depending on the time available, you will be sent or requested to complete a medical history and patient registration form prior to your appointment. Print these forms here and bring the completed forms with you to your appointment.

  4. Please bring a list of your current medications including doses, and all x-rays and test results that relate to your current problem.

  5. Co-payments are required to be paid at the time of your visit. This is a contractual requirement with your insurance provider.

  6. Please bring your insurance card, address of your insurance company, your social security number, and your employer's address. If you are not the subscriber of the insurance, you will need to provide this same information for the subscriber.

  7. For work-related injuries, you will need to provide a Worker's Compensation claim number. Until a claim number is provided to our office, your account will be handled as self-pay.

  8. Please bring the names and addresses of any referring physicians or physicians involved in your care.
