A: Normal turnaround time is 7-10 business days. Depending on the type of form, there may be a fee.

A: Check our list of participating insurance networks. We accept most insurance plans, but you should check the level of benefits with your insurance provider. Many insurance providers have web sites where you may search to verify benefits, but many of these are not kept up-to-date. If in doubt, call the customer service number listed on your insurance provider card and give them our tax id number, 31-1339186.

Q: Do you bill my insurance company?
A: Yes, we will submit a claim to your insurance company, but any co-payments, co-insurance, etc. are expected at the time of service.
