Prescriptions and Refills

  1. Prescriptions and refills will only be issued between the hours of 9:00 a.m. and 4:00 p.m., Monday through Friday.

  2. When calling the office to request a change in medication, please have the phone number of your pharmacy available.

  3. To obtain a refill, please contact your pharmacy and they will contact our office for authorization.

Fees, Payments and Insurance Procedures

  1. We accept cash, personal checks, VISA, MasterCard and Discover as methods of payment.

  2. Questions regarding your particular insurance coverage should be discussed with your insurance provider.

  3. If you are scheduled for surgery, please check your insurance coverage to determine if you are required to obtain pre-surgical authorization or a second opinion.

Q: What are your office hours?
A: Our office hours are between 8:30 a.m. and 5:00 p.m., Monday through Friday.

Q: What times do you schedule appointments?
A: Each physician's schedule is different; appointments are generally available between 8:00 a.m. and 5:00 p.m., Monday-Friday.

Q: Do your physicians go to multiple locations?
A: Yes. Our main office is located at 222 Piedmont (the building is known as the MAB or Medical Arts Building). Our satellite offices are not staffed full-time, so please call our main number (513-475-8690) to schedule appointments at any location.

Q: Are your physicians board certified?
A: Drs. Archdeacon, Asghar, Colosimo, Kenter, Le, Stern and Wyrick are all board certified in Orthopedic Surgery.

Q: How long does it take to get a new patient appointment?
A: Appointments for general orthopedic care are usually available within 2-3 business days. However, there can be a longer wait time to see a spine surgeon.

Q: It's been more than a year since my last visit. Is there anything special I need to do?
A: If it has been a year since your last visit,

  • You will need to complete a medical history and patient registration form prior to your appointment.
  • Please bring a list of your current medications including doses, and all x-rays and test results that relate to your current problem.
  • Co-payments are required to be paid at the time of your visit. This is a contractual requirement with your insurance provider.
  • Please bring your insurance card, address of your insurance company, your social security number, and your employer's address. If you are not the subscriber of the insurance, you will need to provide this same information for the subscriber.
  • For work-related injuries you will need to provide a Worker's Compensation claim number. Until a claim number is provided to our office, your account will be handled as self-pay.
  • Please bring the names and addresses of any referring physicians or physicians involved in your care.

Q: Do you have an 800 number?
A: Yes, it is 1-800-711-1130.

Q: How do I obtain a copy of my medical records?
A: We need a HIPAA-compliant written authorization signed by the patient to release medical records. Once a release and payment are received, normal processing time is approximately 7-10 business days.

Q: Is there a charge to receive copies of medical records?
A: Yes, University Orthopaedics and Sports Medicine follows the Ohio state law for the processing fees. Once a HIPAA compliant medical release is received, the total expense will be provided for payment prior to processing the medical record copies.

Q: Where should I go for physical therapy?
A: You should check with your insurance company to find out which providers are included in your coverage. You can make your choice from your insurance listing.

Q: How do I obtain copies of my x-rays?
A: It depends on where your x-rays were taken. Contact our Medical Records office at 513-475-7278 for assistance in locating/requesting copies of your x-rays. There is a charge of $10 per sheet for copies. A medical record request will need to be completed, which has a 7-10 day processing time.

Q: How long will I have to wait to be seen by the doctor?
A: While we try our best to see you as close to your appointment time as possible, emergencies and unexpected delays may arise. However, once it is your turn to be seen, our physicians will spend the time needed to provide you the best possible orthopedic care.
